The Options program provides home and community-based services to enable consumers to remain in their home. Services are provided on a sliding-fee scale to those who qualify.
In order to participate, call the agency at 724-282-3008 option 3 to discuss a referral. Individuals will be assessed by the department. After the assessment, if eligible for services, a care manager will develop a plan of care that meets the needs of the individual. The department contracts with care providers to provide the services.
Eligibility
- 60 years of age or older
- Reside in Butler County
- Verify financial information such as income and assets in order to determine cost sharing
